Online Information Gathering
Information collected online is often defined as being either anonymous or personally identifiable. Anonymous information cannot be tied back to a specific individual. Personally identifiable information lets us know the specifics of who you are, such as your name, e-mail or postal address, phone and fax numbers. The choice of how much of your personally identifiable information you choose to disclose to the Center for Advancing Health is up to you.
The Center may use the information that we collect to conduct surveys of our subscribers. Other than required demographic information, we will not ask for personally identifiable information. The Center reserves the right to perform statistical analyses of the responses to such surveys. Whenever possible we will make the information we collect about our subscribers available to our subscribership as anonymous information.
On certain pages of our site, we give you the option of providing us with names, addresses, fax numbers, e-mail addresses, demographic and other personal and financial information. This information will be used to send requested materials, a tax receipt (if a donation is made), or purchased items. We do not gather this more personal information without your knowing and active permission and participation.
We will never intentionally disclose any personal information collected from you on our site to any third party without having received your permission. In other words, without such permission, we will not sell, rent, loan or give personally identifiable user information collected from you on our site to marketers or any other third parties. The only exception to this is if the Center is required by law enforcement or judicial authorities to provide personally identifiable information to the appropriate governmental authorities. However, as with any Internet transmission that is not encrypted, the risk of disclosure to unintended third parties exists.

You may find links to other sites or services on the Center's site. We do not control the content or links that appear on these sites. The fact that we have made these links available to you is not an endorsement or recommendation to you by the Center. Other companies or agencies linked to this site may collect information about you when you view or click on these sites. We cannot control this collection of information. You should contact these content providers directly if you have any questions about their use of the information they collect. These sites and services may have their own privacy policies, or no policy at all. If you have provided us with any personal information during a prior visit to or use of the Center's website, you may request that we delete your personal information in its entirety from the databases maintained in connection with our site by sending an e-mail. General inquiries may be sent to firstname.lastname@example.org and will be checked occasionally by former CFAH staff until June 2015.
It is the Center's policy that personal information, such as your name, postal and e-mail address or telephone number, is private and confidential. Accordingly, the personal information you provide is stored in a secure location, is accessible only by designated staff, and is used only for the purposes for which you provide the information.
Like many web sites, we use "cookie" technology. A "cookie" is a piece of data stored on the user's machine that enables the browser to store and recall information and only has meaning to the Center's web site.
